The Case Manager is responsible for educating hospital patient's bedside about ExactCare Pharmacy services. The Case Manager will then visit the discharged patient's home to conduct patient assessments and ensure the safety and compliance with each patients' medication management.
Essential Duties & Responsibilities:
Educate patients in the hospital about ExactCare Pharmacy and services to grow assigned territory.
Keep lines of communication open between assigned Account Manager and Director of Sales and Marketing to address potential issues that may/may not arise.
Manage the patient assessment process to grow business and maintain referral relationships.
Conduct bedside assessment meetings with 175 patients and/or their families, health professionals, etc. per week to assess and review their situation related to their medication.
Responsible for accurate data entry in compiling all patient medication with the appropriate prescription information to ensure the patient will receive the exact medication at the exact time.
Maintain accurate records and prepare reports for the consistency of information to be shared to the New Patients' Department.
Knowledge, Skills & Abilities:
Experience working with hospitalized senior, mental health, low-income population.
Excellent problem solving ability.
Highly team oriented and the ability to help others succeed.
Very organized with strong attention to detail.
Excellent with multi-tasking.
Self-Starter with little to no supervision needed.
Excellent communication skills; oral, written, facilitation and presentation.
Proficient in MS Office Suite.
Passion to help people and enrich their lives.
Valid driver's license in good standing.
Travel required throughout assigned territory.
Required Education, Experience, Certification, Licenses:
Bilingual Spanish/English, preferred.
Bachelor's degree preferred; Social work, Public health or an equivalent field, preferred.
3+ years of experience in social work/LPN field, preferred.
Licensed LPN/LSW, preferred.
