* General Summary of Duties: The Clinical Manager will have direct management responsibility and is accountable for the safe, efficient and cost effective care of patients. The Clinical Manager is responsible for ensuring that care and services are delivered appropriately in accordance with state and federal regulations, as well as the supervision of clinical personnel and other professional and paraprofessional staff as indicated.

Supervision Received: Reports directly to Vice President of Clinical Services.

Supervision Exercised: Responsible for supervision of all disciplines within the healthcare team. Directs and delegates licensed and unlicensed personnel.

Typical Physical Demands: Requires prolonged sitting, requires some standing, bending or stooping. Requires eye-hand coordination and manual dexterity. Requires the ability to lift a minimum of 30 pounds. Requires the ability to ascend/descend stairs. Requires the ability to distinguish letters, numbers and symbols. Requires corrected vision and hearing to normal range.

Typical Work Conditions: Work is performed in an office environment. Contact with Agency staff and community. Required to perform home supervisory visits as part of performance review process.

Essential Duties: (This list may not include all of the duties assigned.)

1. C4, F4 Oversees the assignment of care, clinical integrity of practice and flow of documentation through direct and indirect supervision of team members.

a) Ensures the coordination of patient services are performed with attention

to timeliness and quality care delivery.

b) Ensures compliance with federal, state and Agency requirements with

attention to clinical and financial outcomes.

c) Demonstrates knowledge of and implements HR policies and procedures

for performance review and disciplinary processes with attention to

Agency timelines.

d) Demonstrates leadership abilities. Makes decisions with confidence and in

support of Agency's missions and goals.

e) Utilizes available management data and tools to monitor clinical operations

efficiently.

2. C4, F3 Monitors key clinical and financial outcomes of individuals and team via HH Gold and other management tools.

a) Assures timely completion of all Agency documentation requirements are

met.

b) Ensures team clinicians practice according to Best Practice Standards.

c) Establishes and maintains clinician OASIS assessment proficiency at 85%,

identifying opportunities for staff development.

d) Supports clinician in coordinating care throughout the episode.

e) Fields triaged phone calls from families, patients and physicians as needed.

f) Holds employees accountable to defined expectations of productivity and quality

outcome measures.

3. C3, F3 Completes evaluations of all clinical staff through both record review and annual field visits with new and incumbent staff.

a) Addresses performance concerns and implements plan for improvement as

necessary.

b) Works in conjunction with the PCC to determine staff educational needs.

c) Participates in joint visits as needed.

d) Counsels staff appropriately and in a timely fashion concerning areas of

deficiency as well as areas of excellence.

e) Addresses team risk management issues and resolves.

4. Supports Agency goals and objectives:

a) Assures adequate staffing and monitors productivity to ensure continuity of care.

b) Monitors patient and referral source satisfaction. Promotes customer service

atmosphere.

c) Holds staff meetings on a regular basis and as needed; maintains minutes of

meetings and communicates appropriate information to staff.

5. C2, F2 Participates in own professional growth and development:

a) Attends a minimum of 12 hours of in-services per year.

b) Participates actively in self-appraisal and the formation of a plan of self-development.

c) Knowledgeable of Agency policies and procedures.

d) Seeks appropriate supervision.

e) Adheres to Agency Corporate Compliance Plan and HIPAA Privacy Plan as per

written agreement.

Ancillary Duties:

1. C4, F1 If other resources are not available, may provide intermittent nursing service to patients and families.

2. C4, F1 Participates in Board level activities as requested.

3. C4, F1 Participates in supervisors on-call extended hours coverage.

4. C2, F2 Performs other duties as assigned.

HIPAA and Privacy: Properly direct any complaints and/or grievances as indicated per VNHSC policy. Comply with HIPAA regulations, specifically the Privacy and Security Rules. Protect personal health information (PHI), electronic personal health information (EPHI) and financial information by limiting the use and disclosure of health information and financial information to the minimum amount necessary with only those who must know how to get the job done. Follow the department's specific policies and procedures to protect health information. Help one another to ensure confidentiality at all times.

Knowledge, Skills and Requirements: Knowledge of and ability to apply leadership and managerial principles. Knowledge of supervisory skills. Capable of performing evaluation process for assigned staff. Knowledge of Agency policies and principles. Capable of implementing Agency's goals and directions. Knowledge of communities served. Competent in clinical health principles. Knowledge of state and federal regulations and accreditation standards. Ability to work positively with staff: peers, subordinates and superiors. Ability to function independently, within scope of responsibility. Possess adequate transportation to carry out job requirements. Must show proof of satisfactory automobile insurance with minimum coverage of $50,000/100,000 as determined by Agency policy.

Education: Graduate of an accredited school of nursing with a Bachelor's Degree preferred.

Experience: Five to seven years of Home Health experience is required. Management experience required. Case Management experience required. Excellent working knowledge of OASIS and related outcomes and PPS required. Certification in OASIS preferred.

Certificate/License: Possession of a current license to practice as a Registered Nurse in the State of Connecticut. Possession of an individual Malpractice Insurance policy.
