Conducts intake interviews, completes psychosocial and all required documentation. 2. Completes an Initial Treatment Plan for each patient engaged in treatment consistent with State guidelines and within one month of admission to clinic. 3. Intervenes in patient crisis (concrete or emotional) via short term crisis intervention or via a referral to appropriate resources and/or agency.4. Provides individual therapy, which includes scheduling own appointments and seeing patients on a weekly basis or as otherwise indicated. 5. Presents cases in the clinic case conference.7. Attends and participates in professional associations, activities and programs as requested.8. Participates in all mandated staff meetings.9. Utilizes supervision in order to expand knowledge and in regard to specific job and other areas of job performance. 10. Participates in Continuing Education and In-service Training as required.11. Participates in Quality Improvement studies at the direction of the supervisor.12. Adheres to all personnel, administrative and departmental policies and procedures within the institution.13. Maintains a professional and ethical code of conduct.14. Initiates and participates in smaller case conference when specific problems arise with patients. These conferences may involve the immediate supervisor, staff psychiatrist, manager, and other appropriate personnel.15. Demonstrates the ability to obtain information and interpret information in terms of the patients' needs as appropriate to the ages of the patients served.16. Demonstrates age appropriate competencies as related to the treatment needs of a specific patient target population.17. Demonstrates an understanding of the range of treatment needed as appropriate to the ages of patients served.18. Adheres to policies and procedures as outlined in departmental safety manuals, OSHA Hazcom Manual and infection Control Manual.19. Participates on Quality Improvement teams as needed and adheres to customer service guidelines including telephone performance as established through the implementation of Total Quality Management.20. Adheres to SIUH dress code policies.*ADA Essential FunctionsQualifications Master's Degree in Social Work from a school accredited by the Council on Social Work. Must have New York State LMSW. 1-3 years social work experience in a Psychiatric setting required. Experience in Psychiatric Outpatient Clinic required.
