Job Description:

Regional Hospice and Home Care operates a state of the art kitchen, providing freshly made meals for our hospice patients, as well as family members and staff. We are seeking a part-time cook (max 25 hours per week) with room for growth and benefits for the right candidate. Please send resume and availability. Candidates must meet all local and health regulations and be willing to submit to background check, pre-hire fit for duty exam and drug screen prior to hire. Experience in a skilled nursing care setting preferred, will train. Responsibilities include: Handles food and equipment according to sanitation policies and procedures; Ensure compliance will all applicable state regulations and statutes, and maintain readiness for inspections from the Department of Health at all times. Communicate with Kitchen Manager regarding patient dietary needs, and prepare meals to meet the patient's appropriate nutritional needs in a palatable and attractive manner. Provide a la carte menu preparation for patients, families and staff as needed. Operates equipment in a safe manner; Reports malfunctioning equipment to Kitchen Manager Performs daily or scheduled cleaning duties, in accordance with established policies and procedures; Scrubs and mops floors, cleans and sanitizes equipment; Prepares meals, snacks and beverages for catering needs and for between-meal serviceCustomer Service: Ensure that patient and families receive the highest quality of service in an attentive and responsive atmosphere

Dedicated to its mission of providing care, comfort and compassion to those facing advanced illness in Fairfield, New Haven and Litchfield Counties, Regional Hospice and Home Care has served thousands of area families through its hospice and palliative home care programs. Our agency provides patients and their families with multi-disciplinary care, pain management, spiritual and emotional support, and volunteer programs for dignity and optimum quality of life during end-of-life care, regardless of their ability to pay.

Regional Hospice Choices- An Innovative Hospice Program
Regional Hospice and Home Care has been selected as the only provider in the State of Connecticut to participate in the Medicare Care Choices Model, a model which will provide patients the option to elect supportive care services typically offered by hospice while continuing to receive curative services concurrently. Over 5,000 hospices were invited to apply to participate in this model, and only 140 were selected to participate for a range of 3 to 5 years. Regional Hospice was one of 70 hospices selected nationwide to participate for the full five years of the study.

Center for Comfort Care and Healing:
With the 2015 opening of the Center for Comfort Care and Healing, our patients and their families now also have the option of utilizing these comprehensive, interdisciplinary services in our beautiful, state-of-the-art new Center, a specialty hospital for palliative and end-of-life care. We have the first Center of its kind in the state of Connecticut, and are at the forefront of end-of-life care in the state. While providing patients with the highest quality care 24 hours a day and 7 days a week, we have witnessed families utilizing all of the amenities of our hospice home - spending time with the patient and each other in our welcoming family kitchen, our comfortable living rooms, and our beautiful gardens.

Healing Hearts Center for Grief and Loss
Healing Hearts relocation to the Center for Comfort Care and Healing has brought all of Regional Hospice's programs and services under one roof for the first time. In our Center, we have promoted synergy between hospice and Healing Hearts Center for Grief and Loss, as visitors have discovered there is a convenient, welcoming place they can go for support after the death of their loved one.

Our Service to Patients and the Community:
- We have provided care to patients ranging from age 8 days to age 104, and have supported their families through this difficult time.
- We have admitted patients from seven states, the majority from Connecticut and New York.
- In our first full year, we will serve approximately 1,400 patients a year in the Center for Comfort Care and Healing.
- We will continue to care for approximately 750 patients in their homes, in assisted living residences, and in skilled nursing facilities in our communities each year.
- The 1,900 total hospice patients we will serve each year will also have more than 7,000 family members from throughout the region that will be impacted by our work.
- We will help 1,200 individuals in need of grief support from our Healing Hearts Center.
- Over 15,000 visitors will come to the Center each year to spend time with their loved ones and to participate in educational events on end-of-life care.
