Position reports directly to the AVP of Medical Affairs to coordinate and assist in all clinical functions, duties and responsibilities required to reduce medical costs and improve quality of care for all EP health plan members. RN coordinator must also manage, coordinate and integrate the Interdisciplinary Medical Team experience for all health plan members who have transitioned from acute care settings to enhance quality of life and reduce readmission risk whenever possible. RN must ensure timely information exchange and handoffs across all clinical care interfaces by close collaboration with EP care managers, case managers, transitional care nurses, and interdisciplinary team members. RN must also coordinate regular collaborative meetings with external clinical vendors to ensure effective clinical oversight and appropriate use of limited health care resources.


* RN, BSN preferred, at least 5 years of care management, case management or utilization management experience required
* Proficiency in Word, Excel, Power Point
* Data analytical skills
* Effective communication skills
