Job Description:

The Community Support Network within the Westchester Institute for Human Development in Valhalla, NY is looking for a per diem Start-up/Support Broker to join their Adult Services Team.We are a progressive agency and supportive team seeking a dedicated self-starting individual to work with adults and families using person centered planning to coordinate services and supports to enable individuals with developmental disabilities (who are OPWDD eligible) to access meaningful activities in their daily lives and promote self-determination. Responsibilities include:Interview and consult with the individual and their Circle of Support to create a plan and budget, and sustain and enhance their individualized supports.In collaboration with the same partners and the Financial Management Services provider ensure maintenance of quality services and adherence to the budget. Liaise with the DDSO and provider agencies as required. Requirements/Experience:Bachelor's or Associate's degree in a health or human service field or an RN;One year of experience working with people with a developmental disabilities;Knowledge of NYS Office for People with Developmental Disabilities
