Performs general nursing care and related nursing rehabilitative duties in accordance with established procedures and policies under the supervision of an LPN, RN or RN supervisor responsible for the nursing care of an assigned area. Assignment of unit may change depending on the needs of the residents and the licensed nurse's assessment. As a member of the health team, the CNA shares the responsibility for providing each resident with the nursing care necessary to attain and maintain his/her maximum degree of health.


* Utilizes restorative measures in resident care to promote the resident's highest level of functioning (AROM, PROM and ambulation).

* Assists residents to ambulate. Records distance ambulated.

* Positions residents as ordered (q 2 hrs. or other specified hours positioning) and documenting such tasks.

* Maintains resident proper body alignment; turns and positions resident; assists resident with range of motion exercises.

* Ensures the assigned resident's personal hygiene by administering and/or overseeing bathing, oral care, hair care and nail care including cutting nails of non-diabetic.

* Provides shower or tub bath as care plan outlines.

* Labels and replaces personal care items as needed.

* Ensures residents are toileted in a timely manner.

* Provides skin care and perineal care to all incontinent residents after each voiding and/or defecation.

* Assures residents are appropriately dress in stockings, shoes, proper fitting garments, etc.

* Carries out bowel and bladder management program and records results.

* Provides skin care to prevent decubiti.

* Prepares and administers enemas in accordance with procedures; reports results to nurse.

* Administers catheter care on each shift for residents with foley catheters.

* Takes temperature, pulses and respirations, blood pressures, records and reports measurement to the nurse.

* Collects specimen (urine, stool) as ordered.

* Checks IV site for swelling and/or redness; reports to nurse w/o delay.

* Promotes adequate nutritional and fluid intake to assigned residents.

* Feeds or assists in feeding of residents as needed.

* Encourages residents in self-feeding.

* Reports food and fluid intake and pertinent problems or significant changes in the resident's condition to the RN and documented in the EMR.

* Weigh residents, record results - reporting fluctuations to the nurse.

* Provides fluid as ordered. Measures and records intake and output in EMR.


* Provides for the safety of the residents at all times and documents such activity.

* Monitors chair and bed check alarms. Tests their proper functioning before putting resident in bed or chair and reports problems to the charge nurse.

* Releases restraints as directed by LPN/RN, performs prescribed activity with residents and documents on resident's accountability record.

* Assures that corrective devices are appropriately used; slings, hand rolls and positioning devices.

* Utilizes side rails in accordance with policy.

* Carries out proper isolation techniques as per policy.

* Performs safety rounds and documents per policy.

* Transports the residents to and from clinics, activities, hospitals, etc.

* Assist in admissions, transfers and discharges.

* Unpacks or assists resident to unpack personal belongings upon admission.

* Ensures that all clothing is labeled.

* Upon transfer, discharge or expiration; check resident's belongings against inventory and then packs and sends to the Housekeeping Department.

* Administers postmortem care, packs property and belongings of resident, and transports body to morgue.

* Participates in assigned residents care plan meetings. Reads and follows instructions on care plan.

? Attends scheduled educational in-services. Performs other duties as assigned.


Education: High School Graduate or GED, preferably in a long term care facility.

License Registration: Current NY State certification.

Physical: Mental and physical health to meet demands of the position. Able to stand and work on feet for most of the day. Able to lift and transfer residents from bed to chair, from bed to stretcher, from chair to bed using correct body mechanics at all times. Lifts or carries objects (5-25 pounds) with the use of good body mechanics. Demonstrates ability to push/pull residents in wheelchairs or stretchers with a total weight of up to 250
