Job Description:

WIHD is looking for a full time, entry level LPN who would participate in a broad range of activities designed to facilitate high quality patient care. The ideal candidate will have both clinical and patient care coordination experience as a medical assistant or nurse. WIHD Adult Health Services is a non-profit outpatient clinic located in Valhalla, NY which provides primary care and sub specialty care to teens and adults with intellectual and developmental disabilities. The applicant would be assist primary and specialty providers with the clinical care of patients and assist in planning for follow up care by communicating with, and reiterating the care plan to, family members, patients and agency staff at the end of every visit. The candidate must a current licensure as an LPN in New York State. Ideally, the candidate will have a minimum of three years experience as a medical assistant or one year experience as an LPN in an outpatient setting and have cared for individuals with intellectual/ developmental disabilities (I/DD). Personal experience with individuals with I/DD may be substituted for employment experience with people with I/DD. Prior use of electronic health record and knowledge of medical documentation required by Medicare/Medicaid to support billing and meaningful use are preferred.
