Internal Medicine Primary Care Nurse Practitioner Opening

Provides outpatient primary nursing and medical care in collaboration with the assigned physician(s) and following guidelines and protocols. Reports to the VP, Physician Practices.

Primary Responsibilities:

-obtains a relevant health and medical history

-performs physical examinations and preventive screenings based on age and history

-identifies medical and health risks and needs

-updates and records changes in health status

-formulates appropriate differential diagnoses based on history, physical exam and clinical findings

-orders/conducts and interprets appropriate diagnostic tests

-synthesizes data to determine preliminary diagnoses and therapeutic plan using principles of prevention

-identifies needs of the individual, family or community

-prescribes pharmacological and non-pharmacological therapies

-collaborates with physicians in managing acute and chronic medical needs of patients

-makes appropriate referrals to other health professionals and community agencies

-determines the effectiveness of the plan of care through documentation of patient care outcomes

-reassesses and modifies the plan as necessary to achieve health goals

-maintains accurate, legible and confidential patient records

-performs procedures on patients as appropriate

Experience & Licensing:

At least two years of professional nursing practice, preferably in a community setting.

Active, unencumbered RN and APRN licenses in the State of Connecticut.

DEA and CT Controlled Substance registration
