Family Services Network of New York is a social service agency looking for an energetic team oriented person to join our Children's Health Home team in helping children with chronic conditions, serious emotional disturbance diagnoses and/or histories of complex trauma maintain family unity, overall wellbeing and optimal functioning within the community. The Care Manager will be part of a holistic team that will work collaboratively with children/families and various service providers' to identify the strengths and needs of the child/family and to develop and implement a plan of care to fortify their strengths and appropriately address their needs.

Main Job Tasks and Responsibilities:

The responsibilities of the Care Manager include, but are not limited to:
•Utilize assessment tools to accurately determine clients level of service need and develop a person centered plan of care.
•Dialogue with other providers that contribute to the overall wellbeing and functioning of the child/family in order to identify the specific needs and treatment options available for the clients and their families.
•Serve as a linkage between families and service providers to improve coordination of services and information flow.
•Provide ongoing phone and face to face contacts with clients and their families.
•Provide coordination and linkages to identified needed services.
•Work collaboratively with families and services providers to customize services to best meet members' self-identified and assessed needs within the parameters of available services.
•Complete daily logs for reporting purposes and for weekly review; complete and submit progress notes detailing services provided to the client.
•Provide culturally competent and trauma informed services to children/families enrolled in the program.
•Ensure continuity of care and comprehensive transitional care from service-to service i.e. child to adult, foster care to home, etc.

Skills and Qualifications:

Microsoft Office Skills, Organization and Prioritizing, Professionalism, Problem Assessment and Solving, CANS NY Certified, Effective Written and Verbal Communication Skills, Attention to Detail and Accuracy, Flexibility/Adaptability, Ability to interact with physicians and other service providers in a professional manner

Required: Master's degree with a minimum of 2 years of experience or a Bachelor's degree with a minimum of 5 years of experience working with children with physical and/or mental health diagnosis.

Experience working in the field of child welfare a plus; Experience working in a mental health clinic or a hospital setting a plus; Experience working with electronic health records a plus; Bilingual a plus
